Job Title: Community Health Worker II-Home Health-Maple Ridge

Job Summary:

We are seeking a skilled Community Health Worker II-Home Health-Maple Ridge to join our team at Fraser Health. As a key member of our Home Health team, you will play a vital role in providing high-quality care to our clients in their own homes.

Key Responsibilities:

  • Assist clients with daily living activities to promote independence and self-sufficiency.
  • Provide personal care, housekeeping, and home management services as per the care plan.
  • Perform delegated tasks, such as catheter care and exercise routines, under the guidance of a healthcare professional.
  • Work collaboratively with clients, families, and healthcare teams to establish and maintain a safe and supportive care environment.
  • Participate in quality improvement activities and maintain accurate records and documentation.

Qualifications:

  • Grade 12 education, plus a Home Support/Resident Care Aide certificate or equivalent combination of education, training, and experience.
  • Valid BC Driver's License and access to a personal vehicle for business-related purposes.
