We are seeking a skilled Nurse to join our team at Quality Homemakers Inc. as a Home Care Case Manager.
Key Responsibilities- Manage Home Care Cases: Oversee and coordinate the delivery of high-quality home care services to our clients, ensuring their needs are met and exceeded.
- Provide Nursing Care: Deliver exceptional nursing care to clients in their homes, including administering medications, wound care, and other medical interventions as needed.
- Client Communication: Establish and maintain strong relationships with clients, their families, and caregivers, providing education and support to ensure seamless care transitions.
- Collaboration and Teamwork: Work closely with our interdisciplinary team, including social workers, therapists, and other healthcare professionals, to ensure comprehensive care planning and coordination.
- Documentation and Reporting: Maintain accurate and timely records of client interactions, care plans, and outcomes, ensuring compliance with regulatory requirements and company policies.
- Education: College/CEGEP diploma in Nursing or related field.
- Experience: Minimum 7 months to less than 1 year of experience in home health care or a related field.
- Language: Fluency in English is required.
- Work Hours: 35 hours per week, with flexibility to accommodate client needs.
