We are seeking a skilled Nurse Care Specialist to join our team at Halton Healthcare.


Job Summary:


This is a dynamic opportunity for a compassionate and organized Nurse Care Specialist to provide high-quality patient care in our 36-bed medicine unit.


Responsibilities:



  • Provide safe, timely, and effective care to patients with a variety of medical conditions, including oncology diagnoses.
  • Collaborate with a multi-disciplinary team, including physicians, nurses, and therapists, to develop and implement patient care plans.
  • Assess patient needs, identify potential risks, and develop strategies to mitigate those risks.
  • Communicate effectively with patients, families, and healthcare staff to ensure seamless care transitions.
  • Maintain accurate and detailed patient records, including medical histories, treatment plans, and progress notes.
  • Participate in quality improvement initiatives to enhance patient care and outcomes.

Qualifications:



  • Current registration in good standing with the College of Nurses of Ontario.
  • Recent acute care experience, preferably in an oncology setting.
  • Strong patient assessment and critical thinking skills.
  • Ability to work effectively in a fast-paced environment and prioritize tasks efficiently.
  • Excellent communication and teamwork skills, with a focus on providing exceptional patient care.
