Position Overview

As a Community Support Facilitator, you will serve as the primary liaison for individuals with developmental disabilities and their families. Your responsibilities will include:

  • Assessing eligibility for CLBC-funded services
  • Facilitating access to community resources, informal support, and CLBC-funded services
  • Providing comprehensive information about available community supports and services
  • Addressing crisis situations and resolving conflicts
  • Coordinating various services
  • Developing and implementing personalized support plans with appropriate safeguards
  • Helping individuals and families connect with their communities to leverage their strengths
  • Assisting individuals and families in identifying necessary activities and resources to achieve their goals, while promoting informed decision-making

Qualifications

  • A Bachelor of Social Work or a comparable degree is preferred
  • Experience in the Community Living sector with children, youth, or adults with developmental disabilities and their families is essential
  • A combination of relevant education and experience may be considered

A valid class 5 driver's license and a clean driving record are mandatory.

Work Arrangement This position offers a hybrid work model, allowing for a blend of on-site and remote work after a probationary period.

Travel Requirements Occasional travel may be necessary, including monthly trips and potential overnight stays.

Additional Information
Team: Thompson Cariboo 1
Status: Auxiliary, with anticipated hours of 70 bi-weekly.
