Job Title: Registered Nurse, Older Adult Specialized Seniors Clinic

At Fraser Health, we are seeking a highly skilled and compassionate Registered Nurse to join our team in the Older Adult Specialized Seniors Clinic. As a key member of our interprofessional care team, you will provide person-centered care to patients with complex geriatric needs.

Key Responsibilities:
  • Assess patients referred to the clinic and develop comprehensive care plans in collaboration with the interprofessional care team.
  • Provide direct patient care, including assessments, interventions, and education, to promote optimal health outcomes.
  • Collaborate with patients, families, and caregivers to ensure their needs are met and their voices are heard.
  • Participate in care conferences and councils to ensure seamless care transitions and coordination.
  • Document patient information and care plans accurately and timely, using established practice standards and Fraser Health procedures.
Requirements:
  • Graduation from an approved school of Nursing with current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
  • Two years of recent related acute care experience working with a geriatric population, or an equivalent combination of education, training, and experience.
  • Ability to communicate effectively with patients, families, caregivers, and healthcare professionals using verbal, written, and electronic communication means.
  • Ability to promote person-centered care, demonstrate empathy, and advocate for patients' rights and dignity.
  • Ability to integrate and evaluate data from multiple sources to problem-solve effectively and make informed decisions.
