We are seeking a skilled Licensed Practical Nurse to join our team in Primary Care. As a key member of our interprofessional team, you will provide compassionate patient care and work collaboratively with healthcare professionals to deliver high-quality services.
About the Role:- Provide clinical follow-up and care to patients in accordance with their primary care plan and personal goals.
- Assess, plan, and implement personal and nursing care to patients, including administering medications as per pharmaceutical and organizational policies.
- Work collaboratively with the interprofessional team to achieve optimal health outcomes for patients.
