Position Overview:

Role Responsibilities:

Develop and implement effective Care Plans in alignment with Community Health Services, either independently or in collaboration with Home and Community Care Support Services (HCCSS).

Deliver exceptional nursing services to patients through both in-person and virtual interactions.

Provide care in various settings, including in-home, clinics, retirement homes, or schools.

Communicate and collaborate with patients, family members, and interdisciplinary care teams.

Perform additional clinical duties as necessary.



Qualifications:
  • Registered Nurse (RN) or Registered Practical Nurse (RPN) degree or diploma
  • Current registration with the College of Nurses of Ontario (CNO) in good standing
  • A strong commitment to positively impacting patients' lives
  • Exceptional interpersonal communication skills
  • A valid driver's license and access to a vehicle for travel
  • Excellent verbal and written communication skills in English
  • CPR Certification
  • Current immunization record, including a 2-step TB test
  • Current vulnerable sector check and clear background check to ensure patient safety

Beyond home-based health care, CarePartners actively serves the community through clinics, transitional care units, and provides relief in retirement homes and shared care environments.

Since 2009, our Community Nursing Services outreach program has organized staff-led medical care and clinics in regions with limited healthcare access.
