Role Responsibilities:
Develop and maintain effective Care Plans in accordance with Community Health Services, either independently or in collaboration with Home and Community Care Support Services (HCCSS).
Engage with patients (both in-person and virtually) to deliver exceptional nursing servicesProvide care in various settings, including in-home, clinics, retirement homes, or schools
Communicate and collaborate with patients, family members, and interdisciplinary care teams
Perform additional clinical duties as required
Qualifications:
Registered Nurse (RN) or Registered Practical Nurse (RPN) degree or diploma
Current registration with the College of Nurses of Ontario (CNO) in good standing
A strong commitment to positively impacting the lives of our patients
Exceptional interpersonal abilities
A valid driver's license and vehicle for regular travel
Proficient English verbal and written communication skills
CPR Certification
Current immunization record, including a 2-step TB test
To ensure patient safety, a current vulnerable sector check and clear background check are required

Beyond home-based health care, CarePartners also engages with the community through clinics, transitional care units, and provides relief in retirement homes and shared care environments.
Through our Community Nursing Services outreach program, we have been organizing staff-led medical care and clinics in areas with limited healthcare access since 2009.
