Key Responsibilities- Develop and maintain effective care plans for patients receiving community health services
- Provide exceptional nursing services to patients in their homes, clinics, retirement homes, or schools
- Communicate and collaborate with patients, family members, and other healthcare professionals to ensure seamless care
- Perform other clinical duties as required to meet the needs of our patients
- Registered Nurse (RN) or Registered Practical Nurse (RPN) degree or diploma
- Current College of Nurses of Ontario (CNO) registration in good standing
- Strong commitment to delivering high-quality patient care and making a positive impact in the community
- Excellent interpersonal and communication skills
- Valid driver's license and vehicle for regular travel
- CPR certification and current immunization record, including a 2-step TB test
- Vulnerable sector check and clear background check
