Unit Overview

The Community and Population Health Services (CPHS) division is dedicated to enhancing the integration of health and social services from hospital settings to community environments. This program is tasked with fostering innovative collaborations with peers, community stakeholders, and academic institutions, while providing expert guidance in both pre- and post-hospital transitions.

CPHS oversees HHS' strategies for Population Health, Bundled Care, and Alternate Level of Care. The division offers comprehensive support through various roles including Site Administrative Managers, Managers of Transitions, Transitional Care & Discharge Specialists, ALC Navigators, and the Centralized Care and Transition Team, along with programs such as Hospital to Home (H2H) and Integrated Comprehensive Care (ICC).

Role Overview

The Advanced Practice Nurse (APN) within the Community & Population Health Program is registered with the College of Nurses of Ontario in the Extended Class - Adult and operates within the RN-EC scope of practice.

As a vital member of the Centralized Care and Transition Team (CCaTT), the APN collaborates with an interdisciplinary team, the Emergency Department, and other healthcare professionals, employing a patient and family-centered approach. Furthermore, the CCaTT APN is involved in the Virtual Nursing Care Program, supporting patients with COPD and heart failure while adhering to telepractice standards set by the College of Nurses of Ontario and HHS policies. CCaTT APNs deliver direct patient care, including the assessment and management of patients with intricate health needs, development of integrated care plans for seamless transitions from hospital to community, and post-discharge follow-up care.

At Hamilton Health Sciences, APNs play a crucial role in mentoring healthcare professionals, engaging in ongoing education, leading initiatives, and contributing to the enhancement of interdisciplinary care teams. The CCaTT APN collaborates with program leadership to promote operational excellence and sustainability.

Work Schedule

Monday to Friday with occasional weekend and holiday shifts.

Required Qualifications

  • Current Certificate of Registration in the College of Nurses of Ontario Extended Class - Adult
  • Master's Degree in Nursing
  • At least 5 years of recent clinical experience with older adults facing complex health challenges
  • Proven knowledge of chronic disease management frameworks
  • Familiarity with community resources for integrated discharge planning
  • Ability to critically evaluate literature and apply best practices
  • Knowledge in quality improvement and leadership
  • Strong communication and presentation skills
  • Experience in implementing innovative initiatives through change management
  • Excellent problem-solving, organizational, and prioritization skills
  • Critical thinking and interpersonal abilities
  • Adaptability to changing clinical situations
  • Proficiency in information technology

Proof of full COVID-19 vaccination is mandatory for employment consideration.

Proficiency in both English and French is considered an asset.
