St. Peter's Restorative Care focuses on the rehabilitation needs of adults facing multiple medical and/or functionally complex conditions. Each patient has specific goals and is expected to benefit from low-intensity, long-duration interventions delivered by a collaborative interprofessional team. The anticipated outcomes can realistically be achieved within a 90-day stay, with a focus on preparing patients for discharge by helping them attain their highest level of functioning, independence, and comfort. Common patient objectives include enhancing daily living activities, mobility, wheelchair seating, continence, communication, and adaptation to changes in functioning.

Position Overview

The Licensed Practical Nurse delivers patient care in accordance with the standards set by The College of Nurses of Ontario, nursing standards, and policies/procedures at St. Peter's Hospital, as well as current practices in Gerontological Nursing. Reporting to the Clinical Manager or designated representative, the Licensed Practical Nurse provides nursing care under the guidance and in collaboration with the Registered Nurse and/or Case Manager.

Work Schedule

Rotating shifts including days, evenings, nights, weekends, and holidays.

Qualifications

  • Graduate of a Practical Nursing Program and currently registered with the College of Nurses of Ontario.
  • Proven nursing competencies.
  • Basic computer proficiency.
  • Ability to work effectively within an interdisciplinary team.
  • Strong written and verbal communication skills.
  • Capacity to meet the physical and safety demands of the role.

As a condition of employment, proof of full COVID-19 vaccination is required.
