Situated in central Hamilton, St. Peter's Hospital (SPH) specializes in complex continuing care across various inpatient programs, including palliative care, restorative rehabilitation, and complex medical and behavioral care. Our Medically Complex Care unit is a substantial 63-bed facility. Our multidisciplinary team is dedicated to caring for adult patients post-acute care, who present multiple or intricate medical needs, including tracheostomies and complex wounds. Medically Complex patients require targeted interventions to achieve medical optimization, preparing them for a successful transition back to the community. The extended length of stay allows nurses to build meaningful relationships with patients and their families. Abundant educational opportunities are available, and as we host numerous students from various disciplines, there are chances to serve as a preceptor. We foster a supportive team environment.
Position Overview
The Licensed Practical Nurse on the Medically Complex unit delivers patient-centered care and operates within the full scope of practice for RPNs. This role involves acquiring the knowledge and skills necessary to enhance foundational expertise, promoting optimal psycho-social, emotional, and physical outcomes for patients. The RPN collaborates with Registered Nurses, utilizing the College of Nurses of Ontario's three-factor framework, and works closely with other nursing professionals and interdisciplinary team members. Responsibilities may include delegating tasks and elements of patient care to unregulated care providers. Support is provided by the Clinical Manager and other nursing staff, including Registered Nurses, Nursing Case Managers, Nurse Practitioners, Clinical Nurse Specialists, and Education and Development Clinicians.
Work Schedule
Rotating shifts including days, nights, evenings, weekends, and holidays.
Qualifications
- Valid Certificate of Registration with the College of Nurses of Ontario as a Registered Practical Nurse.
- Current Basic Cardiac Life Support (BCLS) certification.
- Gentle Persuasive Approaches (Basics Workshop) preferred.
- Proven nursing competencies.
- Basic computer skills.
- Excellent problem-solving and communication abilities, both written and verbal.
- Demonstrated proficiency in collaborating with patients, families, peers, and multidisciplinary healthcare teams in a professional and compassionate manner.
- Ability to prioritize and organize tasks based on patient needs.
- Capability to meet the physical and safety demands of the role.
- Consistent attendance record.
As a condition of employment, proof of full COVID-19 vaccination is required.
