Role Overview:
The Transitional Support Professional is responsible for assisting individuals with daily living activities and facilitating personal development in accordance with tailored person-centered plans. This role involves collaboration with a diverse team, families, advocates, friends, and community partners.
Key Responsibilities:
1. Support individuals in daily activities and promote personal growth.
2. Collaborate with clinical professionals to gather relevant data and implement prescribed programs and treatments.
3. Maintain accurate records and provide reports on clinical interventions as required.
4. Complete administrative tasks and responsibilities as assigned.
5. Utilize applied behavior management techniques to support individuals effectively, especially in crisis situations, employing verbal de-escalation and physical interventions when necessary.
Qualifications:
- Developmental Service Worker Diploma or Apprenticeship, Child and Youth Care Diploma (CYC), or related education preferred.
- Advanced proficiency in computer applications, particularly MS Word and Excel.
- Flexibility to work various shifts, including days, afternoons, evenings, alternating weekends, and occasional overnight shifts.
