Position Overview:

The Post Majority Support Services Worker plays a crucial role in assisting young individuals transitioning out of care. Under the guidance of the Post Majority Support Services Supervisor, this position focuses on fostering independence and self-sufficiency among young adults who have previously been in care.

Key Responsibilities:

  • Design and implement tailored support programs that facilitate access to culturally relevant resources and services for young adults.
  • Organize and participate in case conferences and meetings with community service providers.
  • Gather and maintain documentation to monitor the progress and achievements of program participants.
  • Assist individuals in setting personal goals based on comprehensive needs assessments.
  • Establish and sustain regular communication with a network of service providers.
  • Engage in weekly meetings with management to assess program effectiveness and participant development.
  • Complete necessary documentation for program reporting and stakeholder communication.
  • Facilitate family reconnections for young adults transitioning out of care.
  • Comply with all organizational policies and procedures.
  • Uphold confidentiality and professionalism in all interactions.
  • Perform additional duties as required.

Qualifications:

  • A Degree or Diploma in Human Services, Social Work, or a related discipline.
  • A minimum of 2 years of experience in human services or a comparable field.
  • Experience working with individuals facing complex challenges.
  • Familiarity with community resources relevant to the populations served.
  • Knowledge of local Indigenous cultures and traditions is advantageous.
  • Ability to maintain a professional demeanor in high-pressure situations.
  • Strong verbal and written communication skills.
  • Excellent organizational abilities, including prioritization and time management.
  • Capacity to thrive in a confidential and dynamic work environment.
  • Proficiency in Cree or Dene is a plus.

Requirements:

  • Clear Criminal Record Check with Vulnerable Sector.
  • Clear Child and Family Services Intervention Check.
  • Valid Class 5 Driver's License and a clean Driver's Abstract.
