Key Responsibilities
The Care Connector operates within a defined framework, assisting patients in navigating the intricate health care landscape by linking them to suitable health care providers and managing waiting lists effectively.
This is a temporary full-time (1.0) position for a Registered Practical Nurse, lasting up to 18 months.
Required Qualifications
- Registered Practical Nurse, maintaining good standing with the College of Nurses of Ontario.
- Strong understanding of primary care services and community resources, along with the roles of various health care professionals.
- Experience collaborating with diverse health disciplines and engaging with the public and interagency contacts.
- Familiarity with the long-term care and health service delivery systems in Ontario.
- Comprehensive knowledge of tools, systems, and databases utilized in patient service delivery and management.
- Exceptional interpersonal, communication, assessment, problem-solving, and decision-making abilities.
- Proficient time management, prioritization, and organizational skills, capable of working independently and collaboratively in a dynamic multidisciplinary environment.
- Proven ability to accurately complete necessary documentation, reports, and forms.
- A valid driver's license and access to a reliable vehicle.
- Proficiency in a Windows operating environment.
- Compliance with our mandatory COVID-19 vaccination policy, requiring proof of vaccination status prior to employment commencement.
Preferred Qualifications
- Knowledge of services offered by Ontario Health atHome.
- Proficiency in French or another second language.
