Palliative Care Nurse Practitioner

Your Opportunity:
The Palliative Care Nurse Practitioner (PCNP) is a highly skilled, autonomous healthcare provider responsible for directing, delivering, and assessing patient care through advanced nursing and medical management expertise. This role encompasses a wide range of nursing knowledge, skills, and judgment, extending beyond traditional Registered Nursing practices. The PCNP plays a crucial role within the Palliative/End of Life Care framework, integrating functions of practitioner, educator, consultant, leader, and researcher.
The primary focus of this position is to provide timely and effective clinical consultative care to patients in various environments, including community, rural, continuing care, and acute care settings. The PCNP will also mentor healthcare team members to enhance palliative care competencies. This role involves leading initiatives to advance interdisciplinary clinical practices, which includes developing, implementing, and evaluating educational programs, participating in research projects, and engaging in collaborative working groups.
Description:
As a Nurse Practitioner (NP), you will deliver a comprehensive array of health services tailored to individuals throughout their lifespan. Your responsibilities will include utilizing clinical skills from both nursing and medical fields to assess, diagnose, order, and interpret diagnostic tests, prescribe treatments, and perform procedures that address the health needs of patients.
Furthermore, you will leverage advanced practice nursing competencies in Clinical Practice, Leadership, Health Systems Optimization, Education, and Research to enhance care quality, promote effective use of health services, and influence healthcare systems positively.
Required Qualifications:
Candidates must have successfully completed an accredited Nurse Practitioner program and possess a minimum of a master's degree. Registration with CRNA on the Nurse Practitioner Registrar, including Provisional Graduate Nurse Practitioner licensure, is essential. Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP) certification is also required.
Additional Required Qualifications:
Preferred qualifications include CNA Certification in CHPCN(C) for Certified Hospice Palliative Care Nursing in Canada, education in Case Management, and training in Meditech and Connect Care systems. Experience in program planning, adult education, interdisciplinary collaboration, and research activities is advantageous. Strong interpersonal and communication skills, along with the ability to work independently and adapt to evolving clinical situations, are essential. The ability to collaborate effectively with all members of the healthcare team, including patients and their families, is critical.
