The Palliative Care Nurse Practitioner (PCNP) is a highly skilled and autonomous professional responsible for directing, delivering, and assessing patient care utilizing advanced nursing and medical expertise. This role encompasses a comprehensive application of nursing knowledge, skills, and judgment, extending beyond the traditional Registered Nursing practice. The PCNP plays a vital role within the Palliative/End of Life Care portfolio, embodying the functions of a practitioner, educator, consultant, leader, and researcher.

The PCNP is dedicated to enhancing frontline nursing practices and fostering advanced nursing roles within the palliative care consultation team. A significant portion of this position involves providing timely and effective clinical consultative care to patients in various environments, including community, rural, continuing care, and acute care settings. The PCNP is also responsible for mentoring healthcare team members to elevate palliative care competencies.

This position includes leading initiatives to advance interdisciplinary clinical practices in palliative care, which involves developing, implementing, and evaluating educational programs, participating in working groups, and engaging in research projects related to Palliative Care. Travel within the designated healthcare zone is a requirement of this role.

Description:

As a Nurse Practitioner (NP), you will deliver a comprehensive range of health services to individuals throughout their lifespan. Your responsibilities will include integrating clinical skills from both nursing and medical fields to assess, diagnose, order, and interpret diagnostic tests, prescribe treatments, and perform procedures that address the health needs of patients.

You will leverage advanced practice nursing competencies in areas such as Clinical Practice, Leadership, Health Systems Optimization, Education, and Research to enhance care quality, promote the effective use of health services, and influence healthcare systems positively.

Required Qualifications:

Candidates must have successfully completed an accredited Nurse Practitioner program and possess a minimum of a master's degree. Registration with the Nurse Practitioner Registrar, including Provisional Graduate Nurse Practitioner licensure, is mandatory. Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP) certification is also required.

Additional Required Qualifications:

Preferred qualifications include CNA Certification in CHPCN(C) for Certified Hospice Palliative Care Nursing in Canada, Case Management Education, and training in Meditech and Connect Care. Experience in program planning, adult education, interdisciplinary collaboration, and research activities is advantageous. Exceptional interpersonal and communication skills are essential, along with the ability to work independently and adapt to evolving clinical situations. Collaboration with all members of the healthcare team, including patients and their families, is a critical aspect of this role.
