About the Opportunity

We are seeking a highly motivated and experienced Senior Navigator to join our team at Shannex Incorporated. As a key member of our Nursing Home Without Walls team, you will play a vital role in supporting the development and execution of a community outreach program for seniors aged 60+.

Your primary responsibility will be to ensure that seniors and their families have access to appropriate services and information related to aging in place. You will also provide social health initiatives to counter social isolation and loneliness among seniors and their caregivers.

Key responsibilities include:

  • Developing and implementing evaluation tools and reports on initiatives carried out
  • Providing health promotion opportunities and disseminating information that supports aging in place
  • Examining the capacity of community-level approaches to address health inequalities
  • Supporting interventions for aging in place and building capacity of social contacts
  • Contributing to determining best practices for aging in place by relying on infrastructures already present and available in most communities

We are looking for a candidate with a strong understanding of aging, age-friendly communities, and resources as well as issues relating to the older population. If you have a college or university education in social work, gerontology, health care professionals, project management, business management, and/or health and well-being studies related to the position, we encourage you to apply.
