Job Summary:

We are seeking a highly motivated and experienced Senior Community Navigator to join our team at Shannex Incorporated. The successful candidate will be responsible for supporting the development and execution of a community outreach program for seniors, with a focus on promoting healthy aging and age-friendly communities.

Key Responsibilities:

  • Support the development and execution of a community outreach program for seniors, with a focus on promoting healthy aging and age-friendly communities.
  • Ensure that seniors and their families/friends/neighbors have access to appropriate services and information related to aging in place.
  • Provide social health initiatives to counter social isolation and loneliness among seniors and their caregivers.
  • Increase knowledge on health-related issues important to aging in place and healthy aging for seniors and their caregivers.
  • Empower the local community to respond to the needs of an aging population.
  • Oversee and manage the Fredericton Nursing Home Without Walls team.
  • Support the evaluation of how social and built environments can foster healthy aging by creating and implementing evaluation tools and reports on initiatives carried out.
  • Develop programs that address social isolation.
  • Provide a supportive community environment for seniors and their caregivers.
  • Examine the capacity of community-level approaches to address health inequalities.
  • Build capacity of social contacts so that vulnerable seniors can safely remain at home.
  • Provide opportunities for members of the community to collaborate with the nursing home to support seniors in their community.
  • Contribute to determining best practices for aging in place by relying on infrastructures already present and available in most communities.
  • Respect the contributions of seniors, their families, and the community.
  • Encourage seniors to take charge of their health through wellness and lifelong learning.
  • Encourage independence and choices for seniors.
  • Enhance collaboration and develop an age-friendly community approach.

Requirements:

  • College or University education in social work, gerontology, healthcare professionals, project management, business management, and/or health and well-being studies related to the position.
  • English is required, and French is a strong asset.
  • Good knowledge of aging, age-friendly communities, and resources as well as issues relating to the older population.
  • Thorough understanding of performance measurement and evaluation.
  • Thorough understanding of project management.
  • Experience in engaging people through assessments, and/or group work is an asset.
