Patient Care Coordinator

LMC Healthcare (LMC) stands as Canada's foremost specialist provider in diabetes and endocrinology. We are dedicated to revolutionizing diabetes management by enhancing accessibility, comprehensiveness, and patient-centered care. With 11 multidisciplinary centers of excellence across two provinces, our team of over 60 Endocrinologists, many recognized nationally for their expertise, is supported by a highly skilled interdisciplinary team.
We are expanding our team and seeking a Medical Assistant to join us on a permanent full-time basis. This role is integral to our operations, ensuring smooth patient flow and support within our clinical environment.
Key Responsibilities:
- Manage patient flow in examination and treatment areas to optimize facility use and physician schedules.
- Ensure accurate documentation, including patient information and forms, and coordinate with medical laboratories and physician offices as needed.
- Prepare patients for physician evaluations by measuring vital signs such as pulse rate, temperature, blood pressure, height, weight, and BMI, accurately recording this information.
- Gather comprehensive medical and social histories from patients.
- Review medication lists for accuracy and completeness.
- Facilitate coordinated care among the entire LMC care team, including endocrinologists, diabetes educators, pharmacists, chiropodists, optometrists, researchers, and health coaches.
- Prepare examination rooms with the necessary medical supplies.
- Download data from various glucose meters, continuous glucose monitors, and insulin pumps.
- Utilize electronic software for downloading investigations.
Qualifications:
- Experience in a medical office setting and/or a diploma as a Medical Office Assistant.
- Previous experience as a Personal Support Worker or in a Medical Laboratory is advantageous.
- Background as a Medical Assistant (MA) or Practical Nurse is a plus.
- Proficient in Electronic Medical Records systems, with knowledge of Accuro being an asset.
- Strong understanding of medical terminology.
- Prior experience as a medical assistant and proficiency in measuring vital signs, such as manual blood pressure and pulse, is beneficial.
- Excellent interpersonal and customer service skills aimed at assisting patients in accessing services to enhance their health.
