Work Schedule:
The selected candidate will engage in 8-hour shifts, including both day and evening hours, Monday through Friday. Please note that scheduling may be adjusted based on operational requirements.
Compensation:
Salary Range: $71.35 per hour.

Key ResponsibilitiesThe Nurse Practitioner (NP) will deliver comprehensive primary care services to unattached patients aged 6 months and older within our designated service area. This role includes a leadership component focused on innovative care pathways for high-risk pediatric and adult patients, as well as initiatives to reduce emergency department visits.
The NP will conduct medical assessments, treatment plans, health education, and disease prevention strategies, providing support for complex medical cases and frail elderly patients. Additionally, the NP will assist with primary care requests from various patient demographics, including palliative care and cancer screening.
Collaboration with an interprofessional team is essential, promoting best practices in geriatric, pediatric, and palliative care through direct patient engagement and consultation. The NP will work closely with medical leads and community partners to establish priorities and enhance care delivery.
Qualifications- Master's Degree in a relevant field or in progress.
- Current registration as a Nurse Practitioner (PHCNP) with the College of Nurses of Ontario.
- Demonstrated expertise in primary care management is preferred.
- Membership in professional nursing associations is advantageous.
- National certification in specialties such as Gerontology or Pediatrics is preferred.
- Advanced clinical knowledge and application of evidence-based practices.
- Strong interpersonal, coaching, and leadership skills.
- Excellent decision-making and problem-solving abilities.
- Commitment to ongoing professional development.
- Proficiency in health assessment, diagnosis, and therapeutic interventions.
- Ability to autonomously diagnose, order tests, prescribe medications, and perform procedures within the scope of practice.
- Effective communication skills and the ability to engage with diverse populations.
- Experience in program design and project management.
- Self-motivated and capable of achieving results independently.
- Strong analytical and proactive problem-solving skills.
- Proficient in Microsoft Office applications.
- Knowledge of nursing standards and practices.
- Demonstrated commitment to a safe and healthy workplace.
