Position Overview

The Nurse Practitioner (NP) operates in an advanced nursing role, focusing on health promotion and delivering exceptional care through the application of advanced clinical knowledge and skills. The NP's practice aligns with the Mission, Vision, and Values of Yee Hong and adheres to the professional nursing standards established by the organization. The role integrates knowledge from nursing, medicine, and healthcare management within a broad framework of advanced practice. The NP's work balances interdisciplinary collaboration with autonomous practice, continuously seeking to enhance care processes through the integration of standards and feedback from various stakeholders, including physicians, staff, residents, families, caregivers, and volunteers.

The NP plays a vital role in enhancing the quality of resident care by serving as an advanced practitioner, educator, researcher, and leader. As a member of the Yee Hong Centre Management Team, the NP is expected to support the Nursing management team in performance evaluations and reviews. The focus on each aspect of the role may shift over time based on the evolving priorities of Yee Hong Centre. The NP is an essential member of the interprofessional team, collaborating closely with Medical Directors and attending physicians to optimize resident care.

Required Qualifications

  • Current Registration with the College of Nurses of Ontario (CNO) in the Extended Class (Primary Health Care or Adult) and entitled to practice, with a Master's Degree in Nursing (MScN) required.
  • Specialty certification in gerontological nursing and experience in gerontology is advantageous; postgraduate certification and experience in palliative and end-of-life care are also beneficial.
  • Preferred experience as a Registered Nurse (EC) in long-term care or geriatric settings.
  • Knowledge of GPA, PIECES, or other seniors-focused training or professional development is an asset.
  • Proficient computer skills, including familiarity with Point Click Care and MS Office suite.
  • Strong critical thinking, analytical, and problem-solving abilities.
  • Excellent verbal and written communication skills, with the capacity to demonstrate empathy and discretion.
  • Flexible team player, capable of building and maintaining positive working relationships.
  • Good understanding of current trends and legislation in long-term care and community services.
  • Fluency in English is required; fluency in Cantonese/Mandarin is an asset.

Working Conditions

  • Light and normal working conditions.
  • Some travel between Centres may be required; use of personal vehicle is necessary.
