Role Summary:
We are in search of a skilled Registered Nurse / Care Manager who possesses extensive experience in patient care and a deep understanding of specific patient populations.
Key Responsibilities:
The Care Manager is tasked with overseeing patient care coordination from the point of admission to discharge. This includes:
- Facilitating seamless transitions of care as patients are discharged from the healthcare facility.
- Ensuring high-quality clinical outcomes while managing costs effectively.
- Securing post-acute services and advocating for patients and their families with both internal and external stakeholders.
- Identifying and addressing potential obstacles to care coordination and discharge planning.
Care Coordination Process:
The Care Manager initiates the care coordination process upon patient admission by conducting thorough assessments. This allows for accurate information capture and the development of a timely discharge plan.
Interdisciplinary Collaboration:
The Care Manager is an essential member of the interdisciplinary care team, participating in rounds, care conferences, and team meetings.
Patient and Family Advocacy:
Acting as a liaison between the hospital care team and the patient/family, the Care Manager balances patient preferences with care coordination needs.
Post-Acute Services:
The Care Manager identifies and recommends appropriate post-acute services, ensuring timely referrals to care providers while coordinating with patients and families.
Discharge Planning:
Ultimately, the Care Manager is responsible for aligning the discharge plan with the patient's medically cleared discharge date and the anticipated length of stay as determined by the payer.
Education and Support:
It is the Care Manager's role to educate patients and families about post-acute services, transitions of care, and available resources.
Qualifications:
To be considered for this role, candidates must possess:
- A valid, active unencumbered Nursing license or a temporary permit approved by the Georgia Licensing Board.
- A Bachelor's degree in nursing is preferred or actively being pursued.
- A minimum of 2 years of recent healthcare experience.
- Ability to meet quality and productivity expectations and successfully complete annual competencies.
