Job Summary:
Halton Healthcare is seeking a highly skilled and compassionate Registered Nurse to join our team as a Patient Care Specialist. As a key member of our healthcare team, you will be responsible for providing high-quality patient care in a dynamic and rapidly changing environment.
Key Responsibilities:
- Provide specialized patient care in an active complex continuing care unit, focusing on medically complex care, slow stream rehabilitative care, reactivation, or palliative care.
- Support patients and their families, assuming responsibility for the coordination of care, patient and family education, and factors that impact customer satisfaction.
- Administer medications and procedures as per established policies and guidelines, complete health assessments, and document observations and changes in patient condition.
- Collaborate with health team members to update individualized care plans, facilitating positive patient care outcomes.
- Fulfill the charge role and provide support to colleagues as needed.
Requirements:
- Current Registration in good standing with the College of Nurses of Ontario.
- Current Heart & Stroke Approved BLS Provider Card upon hire.
- Proven ability to care and manage patients with tracheostomies, enteral/parenteral feeds, CADD pumps, PICC lines, IVs, Wound Management Systems, Fecal Management Systems.
- Completion of a physical health assessment course.
- One year nursing experience in Long Term Care setting.
- Exceptional communications skills, verbal, written, and electronic.
- Demonstrated compassion, patience, and empathy.
- Demonstrated leadership ability.
- Ability to multi-task and prioritize depending on changing needs of patients.
- Ability to work collaboratively in a team environment but also work independently as needed.
- Computer skills/CCRS data entry an asset.
- Evidence of dedication to continuing education.
- Strong interpersonal, organizational, and time management skills.
- Must be able to meet the physical demands of the position.
- Satisfactory employment and attendance record.
