Job SummaryWe are seeking a highly skilled and compassionate Nurse Care Manager to join our team. As a key member of our caregiving team, you will be responsible for delivering exceptional client experiences, leading our caregiving team, and driving business results through client care and caregiver management.
Key Responsibilities- Consult Conversion: Conduct caring consults, assess client needs, and advise on client care, achieving a 90% conversion goal. Develop and promote high-quality, innovative customer service, and act as an advisor in caring consults to ensure clients have all necessary information.
- Client Care Delivery and Experience: Attend first visits with caregivers, ensure client needs and care plans are executed, and set expectations. Conduct supervisory visits to ensure caregiver fit and quality of care, delivering a premium client experience with Net Promoter Scores of 9 or higher. Develop individualized care plans, educate clients on self-care, and monitor risk issues.
- People Leadership: Lead the caregiving team, set clear goals, assign tasks, coach, and provide continuing education. Deliver on our brand promise by leading the client experience, ensuring caregivers provide exceptional care, and account for staff hiring, training, and performance.
- Scheduling Collaboration: Liaise with the scheduling team, communicate client requirements, and ensure client coverage is secured. Assist with scheduling, manage performance, and ensure data integrity in the scheduling software.
- Administration: Maintain excellent documentation practices, complete and accurate care plans, employee files, and client files. Review and update records quarterly, ensure compliance with legislative requirements, and approve visits and premiums for payroll and billing cycles.
- Registered Practical Nurse or Registered Nurse, licensed to practice in the Province of Ontario
- CPR and First Aid Certification
- Car and valid driver's license
- Strong computer skills, ideally proficient in G-Suite and electronic medical documentation (Alayacare experience an asset)
- Exceptional written and verbal communication skills in English
- Intentional organization, planning, time management, and multitasking skills
- Demonstrated experience setting and achieving goals
- Customer service and leadership background considered an asset
- Background or Criminal Record Check with a Vulnerable Sector Search (updated every 5 years for continued employment)
- Negative TB test (updated every 5 years for continued employment)
