Job Title: Care ManagerWe are seeking a highly skilled and compassionate Care Manager to join our team. As a Care Manager, you will be responsible for delivering exceptional client experiences, leading our caregiving team, and driving business results through client care and caregiver management.
Key Responsibilities- Consult Conversion: Conduct caring consults, assess client needs, and advise on client care, achieving a 90% conversion goal.
- Client Care Delivery and Experience: Attend first visits with caregivers, ensure client needs are met, and deliver a premium client experience, impacting Net Promoter Scores of 9 or higher.
- People Leadership: Lead the caregiving team, set clear goals, assign tasks, coach, and provide continuing education, ensuring our brand promise is delivered.
- Scheduling Collaboration: Liaise with the scheduling team, communicate client requirements, and ensure client coverage is secured.
- Administration: Maintain excellent documentation practices, review and update client and staff records, and ensure compliance with national/provincial legislative requirements.
- Registered Practical Nurse or Registered Nurse, licensed to practice in the Province of Ontario.
- CPR and First Aid Certification.
- Car and valid driver's license.
- Strong computer skills, ideally proficient in G-Suite and electronic medical documentation.
- Exceptional written and verbal communication skills in English.
- Intentional organization, planning, time management, and multitasking skills.
- Demonstrated experience setting and achieving goals.
- Customer service and leadership background considered an asset.
Flexibility is required, with availability 7 days a week. A schedule will be provided 2 months in advance as per business needs. Full-time position with on-call evenings, holidays, and weekends.
