Position Overview

Exciting opportunities are available for Nurse Practitioners at the Foundry Vancouver Granville clinic, a part of Providence Health Care. This clinic specializes in delivering comprehensive physical and sexual health services, mental health support, substance use treatment, peer assistance, and social services to young individuals aged 12 to 24 through a collaborative, multi-disciplinary approach.

In this role, reporting directly to the clinic Director, the Nurse Practitioner will provide episodic, specialized, and continuous care. The collaborative team comprises physicians, nurse practitioners, registered nurses, social workers, counselors, occupational and rehabilitation services, peer support, and administrative personnel. Nurse Practitioners at FVG also benefit from shared care consultations with on-site psychiatrists.

The Nurse Practitioner will work closely with patients, clients, and other healthcare professionals to identify and analyze health trends and patterns that impact individuals, families, and communities. This role involves developing and implementing evidence-based strategies to enhance health outcomes and participating in policy-making initiatives that shape health services and practices. Additionally, the position includes engaging in peer and self-review processes to assess service outcomes at various levels.

Qualifications and Skills

Education
Current registration as a Nurse Practitioner with the British Columbia College of Nursing Professionals and relevant clinical nursing experience. Successful completion of the Objective Structured Clinical Examination (OSCE) and Basic Cardiac Life Support Course, Level C. Eligibility for appointment to the Providence Health Care Department of Nurse Practitioners as Medical Staff is required.

Skills and Abilities
- Strong communication and collaboration skills with patients and families regarding health findings, diagnoses, treatments, and self-care.
- Ability to collaborate and refer patients to physicians and other health professionals as needed.
- Proficient in critically assessing health research literature to determine best practices and introduce evidence-based education.
- Capability to recognize population health trends and implement strategies for prevention and health promotion.
- Competence in evaluating planned changes and supervising others in care delivery.
- Leadership skills to work effectively within a team and adapt to dynamic situations.
- Ability to mentor and coach others while addressing legal and ethical issues in patient care.
- Commitment to self-assess performance and pursue professional development opportunities.

Key Responsibilities

- Diagnose and treat patients with undiagnosed conditions within the Nurse Practitioner’s scope of practice, including writing treatment orders and providing emergency care.
- Monitor ongoing patient care, order necessary diagnostic screenings, and interpret investigation reports to inform treatment plans.
- Establish health management priorities, provide follow-up care, and communicate effectively with patients and families regarding health findings and treatment outcomes.
- Collaborate with the healthcare team and social service providers to determine necessary resources for patient care and develop treatment plans.
- Prescribe medications in accordance with regulatory standards and organizational policies.
- Supervise nursing and healthcare personnel, evaluate their performance, and provide necessary education and support.
- Engage in research and quality improvement initiatives to enhance patient care and advance nursing practices.
- Implement health promotion activities for at-risk populations and participate in the education of nurse practitioner students and other healthcare professionals.

Vaccination Requirement
