Job Summary:

The St. Joseph's Health Centre Guelph is seeking a skilled Nurse Practitioner to join our team as a Long Term Care Specialist. This is a regular part-time position, working 24 hours per week.

About the Role:

The successful candidate will work collaboratively with our inter-professional healthcare team to provide clinical care to residents living in Long Term Care homes. This will involve providing direct clinical care, education, mentorship, and linkages to resources across the continuum of care.

Key Responsibilities:

  • Provide clinical care to residents living in Long Term Care homes, including direct care, education, and mentorship.
  • Work collaboratively with physicians, coordinators, and other members of the inter-professional healthcare team to prevent avoidable ED visits and hospitalizations.
  • Act as a link between the hospital and Long Term Care homes to ensure the most appropriate and efficient clinical care provision.
  • Support the development and direction of the Nurse Led Outreach Team program.
  • Act as a clinical consultant to the inter-professional team in Long Term Care homes, including provision of direct clinical care for acute and episodic conditions.
  • Utilize in-depth knowledge to meet the needs of Long Term Care residents while improving access to quality care.
  • Support the implementation of recommendations or treatment plans established in consultation with attending physicians.
  • Collect statistics related to education and consultation services.
  • Remain current with clinical advancements and network with peers for benchmarking, best practices, and problem-solving.
  • Contribute to the achievement of the Nurse Led Outreach Team goals and annual work plan.

Requirements:

  • Master's Degree in Nursing/Nurse Practitioner (MNNP) or Masters of Science in Nursing with successful completion of a post-Masters Acute Care Nurse Practitioner Certificate program from a recognized school of graduate studies.
  • Current registration with the College of Nurses of Ontario (CNO) as a Registered Nurse.
  • Extended Class (RN (EC)), Nurse Practitioner (Adult) or (Primary Health Care) required.
  • Prior experience as a Nurse Practitioner in the community and/or Long Term Care home in the areas of geriatric mental health/geriatric medicine preferred.
  • A minimum of 3 years' current experience as a Nurse Practitioner is desired.

Skills and Abilities:

  • Motivated self-starter with the ability to work independently and collaboratively as part of a team.
  • Demonstrated time management skills with the ability and flexibility to prioritize multiple tasks in a fast-paced, results-oriented environment.
  • Advanced nursing assessment skills and sound knowledge of clinical therapeutics.
  • Valid driver's license and full-time access to own vehicle.
  • Ability and willingness to travel and work in various urban and rural settings/environments.
  • Excellent oral and written skills and capacity for both self-direction and collaboration within the context of a transdisciplinary team.
  • Excellent computer skills and familiarity with the use of electronic communication devices.
  • Ability to maintain and protect patient confidentiality at all times, demonstrating use of discretion and sound judgment.
  • Demonstrated good attendance record.
