Your Role as a Nurse Practitioner

As a Nurse Practitioner, you will be an integral part of our collaborative, interdisciplinary team dedicated to providing advanced practice nursing to patients and families facing complex situations. You will play a vital role in delivering comprehensive care within our center of excellence for people-centered mental health services.

What You'll Do:
  • Utilize your extensive theoretical knowledge and clinical expertise to provide patient care, including health assessments, diagnosis of medical and cognitive issues, development and implementation of care plans, health counseling, and emotional support in areas such as health promotion, illness prevention, injury management, and complication avoidance.
  • Collaborate closely with consulting physicians to ensure timely assessment and intervention for patients requiring consultations.
  • Support the management of patients by working alongside the interdisciplinary team. This includes conducting comprehensive nursing/medical history reviews, physical assessments, and psychosocial evaluations to understand each patient's unique needs.
  • Utilize established care paths and order sets to guide best practices for patients throughout their journey from admission to discharge.
  • Contribute to the development of individualized treatment plans in collaboration with physicians and psychiatrists. You will work closely with patients, families, and communities to ensure holistic and coordinated care.
  • Create a therapeutic environment that fosters healing and promotes patient well-being.
  • Document and communicate patient beliefs, values, and wishes while upholding ethical and professional boundaries.
  • Demonstrate sensitivity to cultural differences, family dynamics, and individual situations.
  • Empower patients by actively involving them in their treatment decisions and supporting their autonomy.
  • Facilitate positive change guided by patient-defined goals.
  • Assess situations rapidly, accurately identifying potential risks and demonstrating leadership during emergencies.
  • Maintain accurate and comprehensive documentation according to professional standards, clear communication principles, and Homewood Health policies and procedures.
What We're Looking For:
  • 3+ years of nursing experience as a Nurse Practitioner with expertise in mental health and addiction preferred.
  • Current registration in the Extended Class (Nurse Practitioner) with the College of Nurses of Ontario is required.
  • Active membership in relevant professional associations, such as the NPAO and RNAO, is essential.
  • A Master's degree in Nursing or a related field is preferred.
  • Strong clinical skills, critical thinking abilities, and excellent communication and interpersonal skills are crucial.
