We are seeking a highly skilled Registered Nurse to join our High Acuity Unit at Abbotsford Regional Hospital & Cancer Centre. As a key member of our interprofessional care team, you will provide direct patient care by assessing, planning, providing, and evaluating nursing care to patients in an acute medical unit.
Responsibilities- Conduct comprehensive patient assessments, collecting information from various sources, and make informed nursing diagnoses to determine the best course of care.
- Collaborate with patients and their families/caregivers to develop and implement individualized care plans, promoting patient autonomy and dignity.
- Participate in the care and discharge planning process, ensuring seamless transitions and effective communication with the interprofessional care team.
- Coordinate nursing care, prioritize tasks, and delegate responsibilities to support staff, ensuring efficient and high-quality patient care.
- Communicate effectively with patients, families/caregivers, and healthcare professionals, using verbal, written, and electronic means to promote person-centered care.
- Teach patients and families/caregivers about illness prevention, health restoration, and health maintenance, empowering them to take an active role in their healthcare.
- Document assessments, nursing diagnoses, interventions, patient/family responses, outcomes/evaluations, and referrals according to practice standards and Fraser Health procedures.
- Graduation from an approved School of Nursing with current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
- Completion of applicable post-graduate coursework or an equivalent combination of education, training, and experience.
- Current certification in Basic Cardiac Life Support (BCLS).
- Ability to communicate effectively, promote person-centered care, and integrate and evaluate data from multiple sources to problem-solve effectively.
- Ability to teach and instruct patients and families/caregivers about topics essential to their health care and well-being.
- Ability to organize and prioritize work, collaborate with the interprofessional care team, and use factual information to support nursing decisions and actions.
