Job Summary

Fraser Health is seeking a Registered Nurse to join our High Acuity Unit at Abbotsford Regional Hospital & Cancer Centre. As a key member of our interprofessional care team, you will provide direct patient care, assess and plan nursing interventions, and collaborate with patients and families to achieve optimal health outcomes.

Responsibilities
  1. Conduct comprehensive patient assessments, identify nursing diagnoses, and develop individualized care plans.
  2. Collaborate with patients, families, and healthcare teams to promote person-centered care, patient advocacy, and empowerment.
  3. Participate in care planning, coordination, and discharge planning to ensure seamless transitions and optimal patient outcomes.
  4. Provide education and support to patients and families on health promotion, disease prevention, and self-care strategies.
  5. Collaborate with the interprofessional care team to identify and resolve issues, adjust care plans as needed, and address changes in patient condition.
  6. Document patient assessments, nursing diagnoses, interventions, and outcomes in accordance with practice standards and Fraser Health procedures.
Qualifications

Graduation from an approved School of Nursing with current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM). Completion of applicable post-graduate course work or an equivalent combination of education, training, and experience. Current certification in Basic Cardiac Life Support (BCLS).

Ability to communicate effectively with patients, families, and healthcare teams. Ability to promote person-centered care, patient advocacy, and empowerment. Ability to integrate and evaluate data from multiple sources to problem-solve effectively. Ability to teach and instruct patients and families on health promotion, disease prevention, and self-care strategies.
