Support Coordinator for Individuals with Intellectual Disabilities

Position: Support CoordinatorReporting to a Supervisor or Manager, this role involves providing personalized service coordination support to individuals with intellectual disabilities and their families. You will play a vital role in helping individuals achieve their goals and aspirations by connecting them with the right resources and services.
Responsibilities:- Provide tailored service coordination based on the unique needs and goals of individuals and their families.
- Connect individuals with appropriate community services and programs to address their specific goals and aspirations.
- Maintain a comprehensive understanding of available community resources and effectively utilize them to support individuals.
- Champion community inclusion, independence, and self-determination for the individuals we serve.
- Collaborate with colleagues, community partners, families, and individuals to find solutions and address challenges.
- Promote organizational standards and Personal Outcome Measures to ensure high-quality support.
- Identify and report any situations that do not meet organizational standards, ensuring the well-being of individuals is always prioritized.
- Continuously explore and implement innovative approaches to enhance the support provided to individuals and families.
Ideally, you possess a Developmental Services Worker Diploma or equivalent, with at least one year of experience in the field. A University Degree with a minimum of one year of relevant experience will also be considered.
Essential requirements include:
- A valid Ontario Driver's License, access to a reliable vehicle, and insurance.
- A current valid First Aid/CPR certificate.
- Proficiency in using a PC computer environment and MS Office applications.
- Strong interpersonal, communication, problem-solving, and conflict resolution skills.
Bilingualism in English/French or other languages is an asset.
