About the Role

We are seeking a highly skilled and compassionate individual to join our team as a Youth Services Coordinator. In this role, you will be responsible for providing community outreach and support services to youth in need, connecting them with essential resources and services to promote their overall well-being.

Key Responsibilities

  • Develop and maintain relationships with local community organizations and service providers to ensure seamless referrals and support for youth.
  • Conduct outreach and engagement activities in the community, including home visits, group work, and one-on-one support.
  • Assess the needs of youth and develop individualized plans to address their specific challenges and goals.
  • Provide education and support on topics such as mental health, addiction, and life skills.
  • Collaborate with other service providers to ensure a comprehensive and coordinated approach to supporting youth.
  • Maintain accurate records and reports on client interactions and progress.
  • Participate in team meetings and contribute to the development of program strategies and initiatives.

Requirements

  • Diploma in Community Social Service or a related field.
  • Minimum 5 years of experience working with youth in a community outreach or support role.
  • Knowledge of local community resources and services.
  • Ability to work independently and as part of a team.
  • Excellent communication and interpersonal skills.
  • Ability to maintain confidentiality and handle sensitive information.
