Job Summary

We are seeking a skilled Registered Nurse to join our Integrated Home Care (IHC) program as a Case Manager/Interventionist. This dynamic role offers an exciting opportunity to work autonomously, utilizing your full scope of practice to provide high-quality care to clients in the community.

Key Responsibilities
  • Conduct comprehensive nursing assessments to identify client needs and develop effective care plans
  • Provide direct clinical care, including case coordination and management, to ensure optimal client outcomes
  • Collaborate with clients, families, and healthcare teams to promote seamless care transitions and support
  • Monitor services and modify care plans as needed to ensure client satisfaction and effective use of resources
Requirements
  • Completion of an accredited nursing education program
  • Active or eligible for registration and practice permit with the College of Registered Nurses of Alberta (CRNA)
  • Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP) certification
  • Experience working with acute clients and/or long-term clients with a focus on chronic disease management (preferred)
