Alberta Health Services is seeking a Registered Nurse - Case Manager/Interventionist to join our team. As a key member of our Integrated Home Care (IHC) program, you will work autonomously to provide case management functions and direct clinical care to clients in the community.
Key Responsibilities- Conduct nursing assessments and guide care planning to ensure high-quality patient care.
- Provide case coordination and direct care in partnership with clients and their families in their own residence or an alternate care setting.
- Monitor services and ensure they are provided and modified as needed, based on client choice, effectiveness, and authorization.
- Demonstrate proficiency in personal and professional accountability, systems thinking, creative thinking, communicating and building relationships, and organizational and time management skills.
- Travel with your own vehicle and perform manual handling tasks, including lifting, lowering, carrying, pushing, and pulling, as required.
- Completion of an accredited nursing education program.
- Active or eligible for registration and practice permit with the College of Registered Nurses of Alberta (CRNA).
- Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP).
- Completion of the CRNA Medication Management - Chapters and the NextGenU Health Assessment Resources Certification (if not practiced as a Registered Nurse within the last five years).
- Experience working with acute clients and/or long-term clients with a focus on chronic disease management.
- Experience providing case management, client teaching, and professional nursing interventions, including but not limited to Home Parenteral Therapy (HPT), wound care, and ostomy care.
- Experience supporting vulnerable clients, those requiring palliative, mental health, and/or addictions support.
