As a Care Coordinator, your primary responsibility will be to evaluate and identify patient care requirements and eligibility, facilitate access to community resources, and collaborate with patients, their families, and other healthcare professionals.

Whether your work takes place in an office, within the community, or in a healthcare setting, you will be instrumental in delivering connected, accessible, and patient-focused care, supported by a collaborative team of over 8,000 regulated health care professionals.

Your mission as a vital team member will be to assist our patients in achieving better health outcomes at home, while benefiting from our commitment to your professional growth, personal well-being, and work-life balance.

Key Responsibilities

  • Collaborate with patients and their families to assess care needs, determine service eligibility, and create tailored care and service plans.
  • Connect patients with appropriate service providers.
  • Oversee and coordinate the implementation of care plans.
  • Foster supportive relationships with patients and their families.
  • Balance patient preferences and needs with available resources, ensuring respect for their values.

Required Qualifications

  • Active membership with the relevant regulatory body.
  • A minimum of three to five years of recent experience in pediatric health, focusing on children and youth with complex medical needs.
  • Familiarity with Ontario's provincial support systems for children with medical complexities.
  • In-depth knowledge of community services available for children and youth.
  • Proven clinical expertise in providing patient and family-centered care, including critical thinking and ethical decision-making.
  • Demonstrated leadership skills to guide inter-professional teams and collaborate with various stakeholders.
  • Strong interpersonal, communication, assessment, problem-solving, and decision-making abilities.
  • Effective time management and organizational skills, capable of working independently in a dynamic environment.
  • Proven ability to complete necessary documentation accurately.
  • A valid driver's license and access to reliable transportation.
  • Proficiency in a Windows operating environment.
  • Compliance with the mandatory COVID-19 vaccination policy.

Preferred Qualifications

  • Experience working with diverse patient populations.
  • Background in case management or related community services.
  • Ability to communicate in French or another second language.
