In the role of Care Coordinator, you will evaluate and identify patient care requirements and eligibility, facilitate access to community resources, and collaborate with patients, families, and healthcare providers.
Whether you are stationed in an office, working within the community, or in a healthcare setting, you will take a pivotal role in delivering connected, accessible, and patient-focused care, backed by our extensive team of over 8,000 regulated health professionals.
As an integral member of our team, your goal will be to enhance the health of our patients at home, while enjoying our comprehensive support for your professional development, personal well-being, and work-life harmony.
Key Responsibilities
- Collaborate with patients and their families to assess care requirements, determine service eligibility, and create tailored care and service plans.
- Connect patients with appropriate service providers.
- Oversee and evaluate the implementation of care plans.
- Build supportive relationships with patients and their families.
- Balance patient preferences and needs with available resources, ensuring respect for their values and choices.
Required Qualifications
- Active membership with the relevant regulatory body: College of Nurses of Ontario, College of Physiotherapists of Ontario, College of Occupational Therapists of Ontario, Ontario College of Social Workers and Social Service Workers, or College of Audiologists and Speech Language Pathologists of Ontario.
- Three to five years of recent experience in pediatric health, focusing on children and youth with complex medical needs and high-risk conditions.
- Knowledge of Ontario's provincial resources and strategies for children with medical complexities.
- Familiarity with community services and resources for children and youth in the Mississauga Halton area.
- Clinical expertise in delivering patient and family-centered care, including critical thinking and ethical decision-making, while collaborating with various stakeholders.
- Proven leadership skills to support inter-professional teams, including caregivers, parents, and school staff.
- Strong interpersonal, communication, assessment, problem-solving, and decision-making abilities.
- Effective time management, prioritization, and organizational skills, with the capacity to work independently and collaboratively in a dynamic environment.
- Ability to accurately complete necessary documentation and reports.
- A valid driver's license and access to reliable transportation.
- Proficiency in a Windows operating environment.
- Compliance with our mandatory COVID-19 vaccination policy, requiring proof of vaccination prior to employment commencement.
Preferred Qualifications
- Experience working with diverse patient populations, including multicultural, homeless, palliative, acquired brain injury, mental health, geriatric, and pediatric groups.
- Background in case management or relevant community experience.
- Proficiency in French or another second language.
