About the Role

We are seeking a highly skilled and compassionate Care Coordinator to join our team at Home and Community Care Support Services. As a Care Coordinator, you will play a vital role in assessing and determining patient care needs and eligibility, providing access and referrals to community services, and engaging with patients, caregivers, and other healthcare practitioners.

Key Responsibilities
  • Assess patient care needs and determine eligibility for services in collaboration with patients and their families.
  • Develop and implement individual care and service plans that meet the unique needs of each patient.
  • Link patients with service providers and coordinate care plan delivery.
  • Establish and maintain a helping relationship with patients and their families, ensuring their values and preferences are respected.
  • Balance patient needs and choices with available resources, ensuring seamless delivery of care at home and participation in school.
Requirements
  • Membership in good standing with the applicable regulatory body (e.g., College of Nurses of Ontario, College of Physiotherapists of Ontario, etc.).
  • Three to five years of recent experience in children's health with a focus on children and youth with medical complexity and high risk/high utilization conditions.
  • Understanding of Ontario's provincial supports and strategies available to children with medical complexity and other conditions.
  • Experience and knowledge of community services and resources for children and youth within the Mississauga Halton geography.
  • Clinical expertise in patient and family-centred care, including critical thinking, ethical decision-making, and collaboration with various stakeholders.
  • Demonstrated leadership abilities to support inter-professional teams and partners within the circle of care.
  • Excellent interpersonal, communication, assessment, problem-solving, and decision-making skills.
  • Effective time management, prioritization, and organizational skills, with the ability to work independently and co-operatively in a busy multidisciplinary environment.
  • Established ability to accurately complete required documentation, reports, and forms.
  • A valid driver's license and access to a reliable vehicle.
  • Proficient in a Windows environment.
Preferred Qualifications
  • Experience working with diverse patient groups, including multicultural, homeless, palliative, acquired brain injury (ABI), mental health, and geriatrics.
  • Case management experience or recent related community experience.
  • Ability to speak French or another second language.
