About the Role

We are seeking a highly skilled and compassionate Care Coordinator to join our team at Home and Community Care Support Services. As a Care Coordinator, you will play a vital role in assessing and determining patient care needs, providing access to community services, and engaging with patients, caregivers, and healthcare practitioners.

Key Responsibilities
  • Assessment and Planning: Conduct thorough assessments of patient care needs, determine eligibility for services, and develop individual care and service plans in collaboration with patients and their families.
  • Linkage and Coordination: Link patients with service providers, coordinate and monitor care plan delivery, and establish a helping relationship with patients and their families.
  • Resource Management: Balance patient needs and choices with available resources, ensuring patients' values and preferences are respected.
Requirements
  • Education and Certification: Membership in good standing with the applicable regulatory body (e.g., College of Nurses of Ontario, College of Physiotherapists of Ontario, etc.).
  • Experience: 2+ years of recent experience in community health or a related field.
  • Skills: Excellent interpersonal, communication, assessment, problem-solving, and decision-making skills. Effective time management, prioritization, and organizational skills, with the ability to work independently and co-operatively in a busy multidisciplinary environment.
  • Other: A valid driver's license and access to a reliable vehicle. Proficient in a Windows environment.
Preferred Qualifications
  • Experience working with diverse patient groups: Experience working with multicultural, homeless, palliative, acquired brain injury (ABI), mental health, geriatrics, and pediatric populations.
  • Case management experience: Recent case management experience or related community experience.
  • Language skills: Ability to speak French or another second language.
