At Seasons Retirement Communities, we are seeking a highly skilled Registered Practical Nurse (RPN) to join our team as a Care Coordinator. This is a unique opportunity to make a meaningful difference in the lives of our residents and their families.
Job Summary:We are looking for a dedicated and compassionate RPN to provide high-quality nursing care to our residents. As a Care Coordinator, you will be responsible for supervising and mentoring our care team, ensuring that our residents receive the best possible care and support.
Key Responsibilities:- Provide high-quality nursing care to residents, utilizing the nursing process to assess, plan, implement, and evaluate care.
- Supervise and mentor care team members, providing guidance and education to ensure excellent care services.
- Conduct assessments of residents moving into our community and follow up with residents throughout their stay.
- Establish priorities based on individual resident needs and provide person-centered service plans.
- Ensure compliance with Resident Assessments/Service Plans (PCC) and community-specific policies and regulations.
- Risk management by adhering to thorough monthly care-related audits.
- Medication administration (PCC knowledge an asset).
- Current Registered Practical Nurse License and good standing with the CNO.
- Current First Aid and CPR certification.
- Strong verbal and written communication skills.
- Must be prepared to respond to emergency situations.
- Experience working with seniors in a retirement or long-term care facility environment is considered an asset.
- Strong leadership and supervisory skills in relation to both residents and care staff.
- Point-Click-Care (PCC) knowledge is considered an asset.
