About the Role

We are seeking a highly skilled and compassionate Registered Practical Nurse (RPN) to join our team as a Care Coordinator (RPN) - Supervisor at Seasons Retirement Communities. As a key member of our care team, you will be responsible for providing exceptional care services to our residents, mentoring and guiding our Resident Guest Attendants, and ensuring the highest standards of care are maintained.

Key Responsibilities
  • Utilize the nursing process to provide high-quality nursing care to residents, ensuring their physical, emotional, and social needs are met.
  • Supervise and mentor care team members, providing guidance and education to ensure excellent care services are delivered.
  • Conduct assessments of residents moving into our community and follow up with residents throughout their transition to ensure their care needs are met.
  • Establish priorities based on individual resident needs and provide person-centered service plans.
  • Ensure compliance with Resident Assessments/Service Plans (PCC) and community-specific policies, procedures, and regulations.
  • Risk management by adhering to thorough monthly care-related audits.
  • Administer medications as required, with knowledge of PCC an asset.
Requirements
  • Current Registered Practical Nurse License and good standing with the CNO.
  • Current First Aid and CPR certification.
  • Strong verbal and written communication skills.
  • Ability to respond to emergency situations.
  • Experience working with seniors in a retirement or long-term care facility environment an asset.
  • Strong leadership and supervisory skills in relation to both residents and care staff.
  • PCC knowledge an asset.
