Care Coordinator

We are seeking a highly skilled and compassionate Registered Practical Nurse (RPN) to join our team as a Care Coordinator (RPN) - Supervisor. In this role, you will be responsible for providing exceptional care services to our residents, mentoring and guiding our care team, and ensuring the highest standards of care are maintained.
Key Responsibilities- Utilize the nursing process to provide high-quality nursing care to residents
- Supervise and mentor care team members to ensure excellent care services
- Conduct assessments of residents and develop personalized care plans
- Establish priorities based on individual resident needs
- Ensure compliance with regulations and community policies
- Current Registered Practical Nurse License and good standing with the CNO
- Current First Aid and CPR certification
- Strong verbal and written communication skills
- Experience working with seniors in a retirement or long-term care facility environment
- Strong leadership and supervisory skills
