Your Role:
We are in search of a Care Coordinator (RPN) to oversee and deliver top-tier care services within our community. This position involves ensuring a safe and secure environment for residents, their families, and fellow staff members while guiding and educating our Resident Guest Attendants and other team members.
Key Responsibilities:
- Utilizing the nursing process to deliver high-quality care to residents.
- Overseeing care services to ensure excellence and providing ongoing mentorship and education to the care team.
- Conducting assessments for new residents and maintaining follow-up evaluations throughout their transition.
- Setting priorities based on the unique needs of each resident.
- Creating personalized service plans for residents.
- Ensuring adherence to Resident Assessments and Service Plans while complying with community policies and regulations.
- Implementing risk management strategies through regular care audits.
- Administering medications as required, with knowledge of Point-Click-Care (PCC) being advantageous.
Required Qualifications:
- Valid Registered Practical Nurse License in good standing with the regulatory body.
- Current First Aid and CPR certification.
- Strong verbal and written communication abilities.
- Preparedness to respond effectively to emergency situations.
- Experience in senior care settings, such as retirement or long-term care facilities, is preferred.
- Demonstrated leadership and supervisory skills with both residents and care staff.
- Familiarity with Point-Click-Care (PCC) is considered an asset.
