**KEY FUNCTION**:
Provide comprehensive social work services to patients and their families struggling with a mental health and addiction (MHA) illness. Promote optimal coping and wellbeing to mitigate psychosocial distress.

**REPORTING**:
Under the general direction of the Clinical Manager, Acute Inpatient Psychiatry (AIP).

**DUTIES**:

- Provide clinical services to address the specific psychosocial needs of patients and their family members in AIP.
- Provide in-depth psychosocial assessments on patients/families identifying issues relating to the impact of MHA on emotional adjustments and instrumental concerns through periods of crisis and long term adjustments.
- Maintain up-to-date charting/records in compliance with HSN standards, departmental policies and procedures, and the Ontario College of Social Workers and Social Service Workers standards of practice.
- Formulate a treatment plan involving appropriate individuals and agencies within Northeastern Ontario.
- Provide therapeutic clinical intervention and education for individuals, families, and groups including therapy and emotional support.
- Act as advocate for available resources necessary for care and basic living needs.
- Advocate for and implement interventions, service deliveries, and policies that support diversity regarding gender, language, sexual identity, and culture.
- Consult with appropriate team members and work collaboratively in an inter-disciplinary setting.
- Receive and provide peer supervision.
- Supervise and evaluate university level clinical social work practicums.
- Organize and participate in staff meetings, clinical rounds, conferences, workshops and in-services within HSN and in the community.
- Maintain relevant statistical information.
- Develop and implement clinical procedures and standards of work.
- Participate in education and training specific to current, relevant federal and provincial health and safety legislation/standards and guidelines.
- Determine and align improvement projects with HSN’s Strategic Plan; monitor and adjust to achieve goal outcomes.
- Educate and promote health, safety and wellness in the work place.
- Represent the department or program on various committees and in meetings as required.
- Perform other duties as required.

**QUALIFICATIONS**:
**EDUCATION AND TRAINING**:

- Minimum of a Master’s Degree in Social Work from an accredited university.
- Current Certificate of Registration in good standing with the Ontario College of Social Workers and Social Service Workers (OCSWSSW) is required.
- Certification in Cognitive Behavioural Therapy (CBT), Family Interventions Therapy (FIT), Motivational Interviewing (MI), and Mindfulness-based stress reduction (MBSR) is preferred.
- Ministry of Labour “Worker Health and Safety Awareness in 4 Steps” training certificate is required.

**EXPERIENCE**:

- Minimum of three (3) years’ experience working in a mental health and addictions environment.
- Minimum of five (5) years’ of related pratical experience working within a healthcare environment.

**KNOWLEDGE/SKILLS/ABILITIES**:

- Demonstrated knowledge of government legislation, policies and programs affecting the patient population served (i.e. Health Care Consent Act/Mental Health Act and Child Protection).
- Demonstrated working knowledge of general adult and children’s mental health issues, crisis intervention, and impact of cancer trajectory on these issues.
- Demonstrated comprehensive knowledge of community resources and eligibility criteria.
- Demonstrated understanding of government-based income support program.
- Demonstrated strong understanding and sensitivity to end-of-life and palliative care themes for patients and family; ability to identify a Power of Attorney or a Substitute Decision Maker.
- Demonstrated skills in assessment and intervention with individuals, groups and families.
- Demonstrated understanding and experience of family systems and dynamics.
- Demonstrated ability to provide group-based interventions, both psychoeducational and therapeutic.
- Demonstrated knowledge of mobilizing community health resources and enhancing continuum of care.
- Demonstrated knowledge of Indigenous and Francophone culture.
- Demonstrated training, experience or utilization of lean methodology for process improvement.
- Demonstrated ability to independently identify issues, plan improvements, measure success and continue improvement.
