The Outpatient Neurological Rehabilitation program at Health Sciences North (HSN) is seeking a Social Worker Masters to provide rehabilitation programming with a focus on neurological rehabilitation for outpatients, post-discharge from hospital.

The successful incumbent will have the opportunity to participate in a pilot service for outpatient stroke and neurological patients.

You will be responsible for the provision of services including assessment, referral, short-term intervention, locating and arranging resources, and client/family education and provide individual and group therapies to clients and families, including the provision of support with respect to acceptance and loss, psycho-education, and skill building in areas such as relaxation, stress management, problem solving, sleep hygiene, and self-advocacy.
