Education and Training:

  • Successful completion of a Masters degree in Social Work _(BSW with five (5) years recent experience in Eating Disorders will be considered)_
  • Currently registered as a Social Worker with the Ontario College of Social Workers and Social Service Workers
  • Membership in the Ontario Association of Social Workers preferred

Experience:

  • Competencies in assessment, treatment and counselling of patients with chronic disease
  • Recent and relevant experience (minimum 3 years) in the field of Chronic Disease with preference for diabetes experience
  • Understanding of the psychosocial, physical and functional problems endemic to diabetes
  • Experience with information technology (video conferencing, electronic mail, telehealth, website development) for consultation, assessment training purposes
  • Experience in communitybased, outpatient program or regional program preferred
  • Experience in public education, group presentations and teaching, and community mental health service delivery preferred

Knowledge:

  • Knowledge and understanding of principles of diabetes self
- management

  • Proven knowledge and understanding of best practices as it relates to diabetes and associated comorbidities
  • Proven indepth knowledge of psychological factors affecting patients with chronic disease and indepth
  • Knowledge of DSM-IV and psychosocial issues affecting seriously mentally ill clients and those with addictions.

Demonstrated Skills/Ability:

  • Strengths in program planning, development and evaluation
  • Skills in a range of individual, family and group therapies as well as experiential therapies
  • Proven ability to foster and improve relationships between professionals, programs and agencies
  • Demonstrated proficiency in Microsoft Office Suite software, e.g. Word, Excel and Outlook
  • Demonstrated excellent interpersonal and communication skills with the proven ability to communicate effectively with patients, physicians and other health care professionals
  • Proven leadership, problem solving and organizational skills

Personal Suitability:

  • Demonstrated ability to work as a member of a multidisciplinary team, as well as being able to work independently in a fast pace environment with challenging clients and families
  • Valid Driver's License with access to vehicle
  • Demonstrated positive work record and good attendance record
