This position is not eligible to work remotely.

**Main Function**: To assess the social and emotional impact of physical and mental illness or impairment of residents of Pioneer Manor. To influence the social determinants of health that are relevant to the resident by intervening with the resident, family, other residents and staff of Pioneer Manor and the broader community emphasizing the building of existing strengths, modifying risks, and seeking solutions to issues that interfere with optimal quality of life.

**Characteristic Duties**: Under the general supervision of the Manager of Therapeutic Services.
- Assess the social and emotional impact of physical and mental illness or impairment on residents of Pioneer Manor.
- Make recommendations for preservation and enhancement of physical, emotional, and social functioning.
- Review medical records; provide assessments; set goals and objectives for residents on admission to prevent increased disability or dysfunction and/or enhance function and quality of life.
- Communicate with the Power of Attorney/Substitute Decision Maker and interdisciplinary team regarding resident status changes, needs and interventions. Participate in resident and interdisciplinary team conferences as required.
- Document on residents’ medical charts regarding services provided and update care plans accordingly.
- Provide instruction and information to staff, students, residents, and families regarding social work intervention.
- Collaborate with the interdisciplinary team to minimize institutional aspects within Pioneer Manor and counteract the impact of helplessness, hopelessness, loneliness, and boredom.
- Collaborate with the Home’s Behaviour Supports Ontario (BSO) team to support and guide resident care.
- Advocate for residents’ and family needs and encourage resident/family involvement.
- Establish appropriate therapeutic relationships with residents and families for counselling and emotional support.
- Assist residents’ ability to self-direct care to the extent possible through actively involving them in planning and decision-making according to capacity.
- Assist with admission and/or transition adjustment needs of both resident and family.
- Facilitate problem solving and interventions that may arise with complex resident/family needs and challenging behaviours.
- Assist with palliative and end of life matters that present with residents and family.
- Prepare and maintain statistical records, reports and/or other administrative data.
- Collaborate with the broader community in order to integrate and coordinate linkages to provide services or access funding with appropriate external agencies and programs.
- Assist with the completion of forms to related government agencies for financial assistance.
- Maintain clinical competence by mentoring students, completing continuing education and participating in research.
- Assist with managing and organizing the internal transfers as per the resident’s level of care and type of accommodation requested.
- Support, develop and implement innovative programs relevant to the Vision and Mission statement of Pioneer Manor.
- Develop and maintain a thorough working knowledge of CGS’s Safety Manual and the applicable Provincial Legislation listed therein, and of applicable Home manuals.
- Perform other related duties as required.

**Qualifications**:
**Education and Training**:

- Successful completion of a Bachelor of Social Work Degree from a recognized University with Canadian accreditation.
- Membership in good standing with the Ontario College of Social Workers and Social Service Workers.
- Membership with the Canadian Association of Social Workers or the Ontario Association of Social Workers an asset.

**Experience**:

- Minimum of two (2) years of related experience in a health care setting, preferably in a long-term care home

**Knowledge Of**:

- Developmental stages of residents within long-term care.
- The impact of various physical and mental health conditions on the well being of residents and families.
- Current trends and best practices in Social Work and long term care.
- The Occupational Health and Safety Act and in particular its relation to long-term care facilities.
- Applicable legislation and related regulations including regulations governing long-term care facilities under the Ministry of Long-Term Care as they relate to Social Work services.
- Best practices within areas of responsibility.
- Horizontal linkages to other relevant governmental levels and services as well as the private sector.
