**About the Organization**
First Nations Technical Services Advisory Group Inc. (TSAG) is a not-for-profit provider of quality technical service and training for First Nations in Alberta. TSAG’s purpose is to assist First Nations in gaining the tools and knowledge they need to achieve and maintain high standards in technology and services within the community. This is a part-time position of up to 20 hours per week, which includes evenings and weekends and will be located in Edmonton, Alberta. TSAG operates Alberta Indigenous Virtual Care Clinic (AIVCC).
**Purpose & Scope**
The AIVCC seeks one part-time Patient Care Coordinator for compassionate support of Indigenous patients across Alberta. As a member of our team, you will work alongside healthcare providers, families, and other stakeholders to ensure a culturally safe patient care experience. Patient Care Coordinator is clinic-level resource who is supported by physician and nurse champions and other change agents to catalyze clinic-level changes. Patient Care Coordinator helps our member physicians and clinical teams improve patient care by introducing changes, encouraging teams to improve, coaching teams through process changes, and reducing effort of change.
As AIVCC represents a unique model of care, the successful applicant will be responsive to emerging needs of patients and communities with respect to services provided.
The ability to work remotely is a requirement of the position. Funding for this position has been provided through a unique partnership between federal and provincial sources.
**Qualifications and Requirements**
An equivalent combination of education, experience and expertise may be considered.
- Undergraduate degree or equivalent.
- 5 years’ experience in a healthcare related sector (2 years’ experience working in primary care preferred).
- Experience with project management, quality improvement and facilitation.
- Proficiency using Telus MedAccess and Netcare
- Proficiency with Microsoft Office products.
- Valid driver’s license with access to a reliable vehicle for travel.
**Skills**:
- Communicate effectively and constructively, orally and in writing.
- Experience working with Indigenous patients
- Time management and organizational skills
- Able to work as part of a team, demonstrating trust, respect and integrity.
- Self-starter, able to take initiative and anticipate organizational needs.
- Objective and able to represent opinions and evidence fairly.
- Ability to manage conflict with diplomacy and tact.
Duties & Responsibilities or Work Performed:
- Develop professional relationships with clinic teams, physician leaders/champions, RNs, and provincial peers/support programs.
- Gather information from EMR & EHR on patients for physicians and nurses. This may include cancer screening, immunizations records.
- Locate patients that are unattached and seen 3 times or more at our clinic
- Understand clinic team goals for improvement and connect their ideas
- Introduce provincial quality improvement opportunities, programs and support to clinic teams.
- Work with physician and nurse champions, motivate and inspire clinic teams to take action.
- Leverage clinic team member diversity and structured approach to change (Model for Improvement) to achieve clinic goals
- Build quality improvement capacity in clinic teams and other AIVCC staff members.
- Champion and support the implementation of quality improvement measures within clinics to sustain improvements.
- Facilitate measurement and evaluation needs with clinic teams.
- Support and encourage clinic teams to share their improvements and lessons learned with other clinics, zone and province.
