**Role Overview**

Are you an experienced geriatric professional who is passionate about supporting seniors to live at home as long as possible? This is an exciting opportunity for an individual who is eager to uplift the community’s well-being.

The Community Connector is a new role for JFS. JFS is part of the inaugural cohort of the Community Connector program, which is an addition to our already existing partnership with the United Way Healthy Aging Program for Seniors. The Community Connector program will work in collaboration with our Better at Home program and will serve as a crucial liaison between health care professionals, community partners and vulnerable seniors living in Kerrisdale, Oakridge, Marpole, Dunbar and Southlands. Reporting to the Better at Home Senior Coordinator, the Community Connector will play an integral role in bridging the gap between healthcare and social care. This role involves ensuring that seniors receive personalized assistance beyond the scope of our existing programs. This position will play a pivotal part in connecting seniors with the support and assistance they need to be healthy and connected to a community where everyone thrives.

**What You Will Do**
- Build and maintain networks within community and strengthen relationships within the community-based senior serving sector
- Use an asset-based community development approach to identify and mobilize individuals and organizations providing and supporting older adults
- Connect, liaise, and establish partnerships with local health care professionals
- Maintain an active caseload of seniors with short-term needs through referrals
- Complete intake processes through home visits to determine challenges, needs and risks related to health aging and develop routines, structures and resource referrals to reduce risk of frailty
- Supports seniors to access appropriate range of activities and suitable community resources by developing individual wellness plans

**What You Will Bring to the Job**
- Undergraduate degree in social work, gerontology or related experience in social services setting/health care setting; BSW preferred
- Minimum two years’ experience working with seniors
- Excellent written and oral communication skills and ability to clearly explain instructions
- Knowledge of community resources, programs and services
- Valid drivers license and access to vehicle an asset

**Vaccination Policy**JFS requires that all employees and volunteers are fully vaccinated against COVID-19. Anyone hired after January 1, 2022 will be required to show evidence that they have received at least one dose of the vaccine prior to employment, and we will require that they be fully vaccinated within 30 days of their start date.
