We are looking for a motivated **Community Connector Coordinator** to join our **Social Prescription, Community Health and Specialized Programs **team****

**SUMMARY**:
The Community Connector Coordinator plays an integral role in bridging the gap between healthcare and social care services particularly for seniors residing in the City of Burnaby. As a social prescribing professional, the Community Connector Coordinator collaborates and partners the Seniors Services team and with community organizations to access and provide non-medical support to improve the overall health and well-being of older adults in the community. The goal of this position is to assist frail and isolated older adults who are 65+ to support them in healthy aging, and to continue to be independent by connecting them with social engagement and physical activity opportunities as well as providing tips for healthy nutrition and connections to community-based seniors’ services in the Burnaby area.

**POSITION FUNCTION**:
**1. Accountability**
- Be familiar with MOSAIC’S vision, mission, values, strategic plan, and integrated client service delivery approach, program and/or project proposals and contracts, and your role in achieving the goals and contractual outcomes of the department.

**2. Operations**
- Facilitate educational training for seniors and community providers.
- Provide one on one information and education to seniors in co-developing their wellness plan and addressing social barriers and challenges.
- Wellness Plan Development:

- Supports clients to access an appropriate range of activities and suitable community resources by developing individual wellness plans.

a). Provides information regarding appropriate community resources to socially support the participant and their families.

b). Effectively collaborates within the Agency’s and community’s Seniors Services to provide multidisciplinary care for the best interest of the senior.
- Maintains an active client caseload and prioritizes referrals based on needs and communicates with health care professionals and community agencies as needed:
b). Connects, liaises, and establishes partnerships with local healthcare professionals to create and maintain referral pathways.

c). Use related databases and records to enter participants’ information, document wellness plans, and progress reports, and other

administrative duties as required.
d). Provide follow-up/progress notes to health care provider as requested.
- Implement safety precautions when visiting clients in the community.
- Complete intake process to assess strengths, needs, abilities, and risks using motivational interviewing techniques.
